Nobody Has Given You a Straight Answer

Start Here: Do You Have a Rough-In?
Go and look at your basement floor. If you see capped pipes coming out of the concrete, usually one three or four inch stub with a cap plus one or two smaller ones, that is a bathroom rough-in. Most homes built since about 1990 have one.
That one answer moves the price of this job by thousands, which is why the quotes you have been given are so far apart.
If you have a rough-in, the drain is already there and already tied into your main line. From that point it builds much like any other bathroom.
If you do not, there are two honest options. Break the concrete: cut and trench the slab, run the drain to the main stack, tie in, repour. Permanent, invisible, and it works exactly like any other bathroom in the house. It costs more and it is a messy few days.
Or an upflush system. A macerator pump sits behind or below the fixture and pumps waste up to the main line. No concrete work, much cheaper, and it goes in fast. The trade-off is real and we will say it plainly: it makes noise when it runs, and it is a pump, so at some point it will need service or replacing.

Decisions That Shape the Price
Ventilation
Most basement bathrooms have no exterior wall, so the fan has to duct a long way through the joists. A fan sized for a short run does nothing over a long one. This is the single most commonly botched part of a basement bathroom.
Ceiling height against drain slope
Drains need fall to work. Low ceilings and long runs pull against each other, and that is a problem to solve on paper before demolition, not halfway through.
Moisture
Water moves through the slab and the foundation wall whether you can see it or not. A basement bathroom needs a vapour barrier and the right substrate. Building it like an upstairs bathroom is how you get a mould problem in three years.
Backwater valve
Worth discussing. It protects the lowest fixtures in the house from a sewer backup, and your basement bathroom is the lowest thing you own.
Heated floor
In any other room this is a luxury. Down here it is the difference between a bathroom people use and one they avoid. If you are only going to do it once in your house, do it here.
Permits
Adding a bathroom where there was not one usually needs one. We tell you on the visit and we handle it.
